The picture is a little disturbing although I'm sure it was acceptable for 1909. The implication of the message is that the lady on the card looks like Margaret Simpson. Judge for yourself

The sender of the card obviously thinks it looks like her and makes a joke about it.

Overall, some of the message is difficult to follow because of typos and a lack of punctuation. I wonder if anyone had seen Kelly and what connection was there is the picture.
